About the Employer:
S.U.C.C.E.S.S. is seeking dedicated and compassionate Case Managers to join their team. Focused on enhancing the quality of life for individuals and families in the community, S.U.C.C.E.S.S. provides a range of support services aimed at promoting social integration and personal empowerment.

Key Responsibilities:

  • Manage a diverse caseload, providing support and resources to clients.
  • Assess client needs and develop comprehensive service plans.
  • Facilitate access to community resources and services.
  • Monitor client progress and make necessary adjustments to service plans.
  • Advocate for clients and help them navigate complex systems.
  • Maintain accurate records and documentation of client interactions and progress.

Qualifications:

  • Education: Bachelor’s degree in a relevant field.
  • Experience: 1 to 2 years of experience in case management or related roles.
  • Language: Proficiency in English is required.

Workplace Information:

  • This position requires on-site work; remote work is not an option.
